REGISTRATION

Students must complete one of following forms that applies to her or him upon registration:

COMPLETED SCHOOL APPLICATION

1st thru 8th Grade Application

Early Childhood Application

Early Childhood Information Form

Current Student Re-Registration Form

Birth Certificate

Baptismal Certificate

Birth Certificate

Social Security Card

Immunization Records

Non-Refundable registration fee of $40

TUITION INFORMATION


Students currently enrolled in MDP receive
re-registration paperwork before the end of the school year.

MDP provides before and after school care for its students through the CARES program, Monday through Friday, 7:00 a.m. until 6:00 p.m. during the school year.
